The child registration form will take approximately 30-45 minutes to complete.

• Step 2:  Child Health Details + Health Centre/Doctor Details + Emergency Contacts
• Step 3:  Special Requests + Child Authorisations
• Step 4:  Terms & Conditions (Read and Sign)
• Step 5:  Parental Involvement Policy + Emergency Hospital Treatmeant Consent
• Step 6:  ParentMail Form - (permission for information inputted here, to be kept on record with ParentMail®.)
